Undergraduate Registration Forms

Undergraduate Student Information Form - This provides us with information on your co-op address, contact information, and supervisor information.

Undergraduate Co-op Checklist - The checklist informs you of things you need to do before registering and while on your co-op. Check off the aspects that you've done so far, submit a copy to Career Services, and keep a copy for yourself to remind you of the things you should do later on.

Undergraduate Cooperative Education Agreement - This form is an agreement between MTU, the employer, and the Co-op student. It outlines the responsibilities of each party. It must be filled out by the employer prior to the Co-op start date.

Evaluation Forms

Undergraduate Student's Progress Evaluation - To be filled out by the supervisor and the co-op student at the midpoint of each semester in which a student is co-oping.

Graduate Student's Progress Evaluation - To be filled out by the supervisor and the co-op student at the midpoint of each semester in which a student is co-oping.

Undergraduate Student Final Evaluation - This form is to be filled out by students at the end of a co-op assignment.

Supervisor Final Evaluation - To be filled out by the supervisor of the co-op student at the end of a co-op assignment.
